Was ist die Kompressionssonografie und warum wird sie durchgeführt?

Die Kompressionssonografie – auch Venenultraschall genannt – ist ein bildgebendes, schmerzfreies Verfahren zur Untersuchung der tiefen Venen in den Beinen. Mithilfe von Ultraschallwellen kann festgestellt werden, ob ein Blutgerinnsel (Thrombose) vorliegt. Die Untersuchung ist nicht-invasiv, strahlenfrei und kann daher auch bei Schwangeren oder älteren Menschen problemlos durchgeführt werden.

Ziel der Untersuchung ist es, eine tiefe Beinvenenthrombose (TVT) sicher zu erkennen oder auszuschließen. Dabei wird überprüft, ob sich die Venen durch sanften Druck mit dem Ultraschallkopf zusammendrücken lassen – bei einer Thrombose ist dies nicht möglich, da das Gerinnsel die Vene blockiert.

Frühzeitiges Erkennen ist entscheidend, um Komplikationen wie eine Lungenembolie zu vermeiden. Die Kompressionssonografie ist heute die Methode der Wahl zur Thrombosediagnostik.

Bei wem wird die Untersuchung durchgeführt?

Die Kompressionssonografie wird eingesetzt bei Symptomen, die auf eine Thrombose hindeuten können – zum Beispiel eine einseitige Beinschwellung, Schmerzen oder Spannungsgefühl in der Wade, Hautverfärbungen oder Überwärmung. Sie wird auch zur Kontrolle bei Patienten mit Thrombose in der Vorgeschichte, bei Gerinnungsstörungen, nach langen Reisen, Operationen oder bei eingeschränkter Mobilität durchgeführt. Zudem wird sie bei Schwangeren oder Frauen unter Hormontherapie (z. B. Pille) zur Abklärung eingesetzt.

So läuft die Untersuchung ab

Die Untersuchung erfolgt meist im Liegen. Das betroffene Bein wird freigemacht, und auf die Haut wird etwas Ultraschallgel aufgetragen. Anschließend wird mit dem Ultraschallkopf systematisch entlang des Beinvenensystems gefahren – beginnend in der Leiste bis zur Wade. Dabei übt der Arzt leichten Druck auf die Vene aus, um zu prüfen, ob sie sich zusammendrücken lässt. Eine nicht komprimierbare Vene ist ein deutlicher Hinweis auf eine Thrombose.

Zusätzlich kann bei Bedarf eine Doppler-Sonografie eingesetzt werden, um den Blutfluss innerhalb der Vene sichtbar zu machen. Die gesamte Untersuchung dauert in der Regel 10 bis 20 Minuten.

Was sollten Sie vor der Untersuchung beachten?

Für die Kompressionssonografie ist keine spezielle Vorbereitung notwendig. Sie dürfen wie gewohnt essen, trinken und Ihre Medikamente einnehmen. Wichtig ist lediglich, dass das Bein gut zugänglich ist – bitte tragen Sie bequeme Kleidung, die sich leicht hochschieben oder ausziehen lässt. Bei bestehenden Risikofaktoren oder Vorbefunden (z. B. frühere Thrombosen, Gerinnungswerte, Krankenhausberichte) bringen Sie diese bitte zur Untersuchung mit.

In den meisten Fällen kann der Befund direkt im Anschluss besprochen werden. Da die Kompressionssonografie risikofrei ist, kann sie bei Bedarf jederzeit wiederholt werden – z. B. zur Verlaufskontrolle unter Antikoagulationstherapie oder bei erneutem Verdacht auf eine Thrombose.
